To Become Listed on the Referrals page, please follow the instructions below. Please note, you must be a Full OCAMPR Member to be listed on the Referrals page; please no Associate or Student members.

• Download the OCAMPR affidavit and professional information forms and fill them out completely (requires Microsoft Word).
• Two copies of the forms are required. Send one set via email and a second, SIGNED set via regular mail to either the Psychology Coordinator or the Medical Coordinator, whichever appropriate according to your profession. They will review and forward your information for posting on the website and keep a copy of your records.

All attempts will be made to post new information to the website in a timely manner.
